Fort Infirmary is a fort; in the Town region, and expects a character of at least level 1. It is low-risk country — trouble is possible but uncommon. Travel in Desperados Destiny costs energy and takes in-game time, and a route can be interrupted — the danger rating of the road feeds directly into how often that happens. Locations must be discovered before you can travel to them directly, and discovering one for the first time pays experience.
